Spain, November of 2011- the days of the REAP of 2012 will be devoted to the analysis of the problems of coordination between professionals and care levels in patients with serious problems of mental health and illnesses organic with the simultaneously schizophrenic and diabetic patient example.

Presentación

are rare patients who have only a health problem, but the health system is organized as if they were frequent. As a result, the problems of coordination between professionals and levels increase in patients with various health problems.

We must examine these problems of coordination in theory and in practice, and do so from the point of view of social, professional and of patients and their families, in theory and in practice.

Particularly striking are the problems of coordination for patients with serious mental health problems (schizophrenia, autism, Alzheimer’s, etc.) and functional acute and chronic (appendicitis, cancer, diabetes, etc.).
