replacement therapy for patients with disease Renal Crónica.

-the national health system could save 480 million euros to increase the proportion of patients on dialysis Peritoneal (DP) home up to 30%

– A study published in the magazine Peritoneal Dialysis International developed by Spanish specialists evidenced on the basis of data costs, survival and quality of life for kidney patients

Madrid, November 2011- study “ Cost-effectiveness analysis of the Spanish Renal Replacement Therapy Program ” (1), recently published in the magazine Peritoneal Dialysis International, simulates four scenarios which varies the proportion of incidents that do not come by way of emergency but scheduled patients (increasing to 75%) and the number of patients on Peritoneal Dialysis (DP) (increased up to 30%)(, percentage recommended by experts).

Total cost savings varies according to the proposed scenario, but increasing the number of patients in treatment with DP up to 30%, improves survival, the number of patients that can be treated and, in addition, the total cost is less. In fact, up to 480 million euros could save over 15 years.

The study concludes that, by analysing the cost and the efficiency of various forms of renal replacement therapy (TSR), the DP get a favorable outcome and health net benefits are positive. “ Therefore when explores possible scenarios, the best is the one which created consultations pre-dialysis and DP increases 30% achieving greater quantity and quality of life to one lower economic cost ”, says Dr. Rafael Selgas, Coordinator of the support group for the development of the Peritoneal Dialysis in Spain (GADDPE) and Chief of service of Nephrology of Hospital Universitario La Paz. “ Unfortunately, in Spain the DP still underused and too few patients benefit ”, adds.

In fact, according to the article “ peritoneal dialysis is the best cost-effective alternative to the sustainability of the dialysis treatment ” (2), the annual average cost per patient of renal replacement therapy patients is 46,659 euros in hemodialysis and 32.432 euros in home peritoneal dialysisIE “ hemodialysis is 44 per cent more expensive than peritoneal dialysis ”, says Dr. Javier Arrieta, spokesman for the GADDPE and Chief of service of the University Hospital in Basurto (Bilbao).

According to the last row of the Spanish society of Nephrology (SEN), there are currently more than 48,000 patients in Spain that they suffer from chronic kidney disease at an advanced stage and were receiving some form of renal replacement therapy, representing a prevalence of 1.033 patients (the 45.7% in HD)(5% in DP and the 49.2% in transplant) per million population. The economic impact of the TSR in Spain is estimated at 1,829 million euros (3) per year.

The authors of the study conclude that, from a perspective of minimising costs, it would be favourable to increase the number of patients in DP.

Home Peritoneal Dialysis

Peritoneal dialysis is a form of renal replacement therapy home used in patients with chronic renal failure. The Elimination of toxic substances is carried out through the peritoneal membrane and can be carried out manually or automated, at night, while sleeping. The only patient attends hospital to review each one or two months. This helps the patient continue with their work and personal life skills and gives you more freedom and privacy in their treatment.

Peritoneal dialysis presents:

-social advantages, as it allows the patient to keep your work and personal life and provides you with greater independence, intimacy, autonomy and quality of life;

-medical advantages, because it is considered an excellent beginning of dialysis modality, is very suitable for patients who are on waiting list to receive a transplantIt is recommended for all ages and for its continuous nature, is the closest to the normal functioning of the kidneys.

About the GADDPE

The support group for the development of Peritoneal Dialysis (GADDPE) aims to raise the health administration and society in general on the problem of under-utilization of peritoneal dialysis on Spain. The group consists of members of the Group of dialysis Peritoneal the society Spanish of Nephrology (SEN), the Spanish society of nursing Nefrológica (SEDEN), Fundación Renal Alcer Spain (FRAE), the Kidney Foundation Iñigo Alvarez de Toledo (FRIAT) and professors from different universities of medicine and nursing.

“The use misuse of antibiotics may prolong the time of illness.

“ Abuse of antibiotics may prolong the time of disease ”.

– antibiotic resistance is due to poor compliance with treatment or abuse in their consumption

-the European day for the prudent use of antibiotics is raising awareness of the dangers to public health of the antibiotic resistance

Vitoria-Gasteiz, November 2011- use antibiotics responsibly under the medical indications remains a combination hard to bear for many patients in our country according to Leyre Salazarresponsible for the pharmaceutical area of the clinic USP La Esperanza, “ only antibiotics should take prescription and its management must be that indicated by the physician, otherwise, we would be talking about self-medication that eventually would lead to a resistance to the antibiotic and produce negative effects in the treatment of future diseases ”.

According to a European study picked up by the Spanish medicines agency “ Spain lies between countries with a higher degree of self-medication with antibiotics in Europe and gathering them at home ”, reality resulting in a bacterial resistance of the body that would lead to a lengthening of the time of the illness of the patient.

An antibiotic resistance occurs when the antibiotic “ is no longer effective against the bacteria that is designed, situation caused mainly by the poor implementation of scheduled treatment or an abuse of antibiotics ”, as it is the case of their use to treat diseases that do not require their use, as the influenza-like processes.

In this sense, “ will be infections for which we need more doses, or another more powerful antibiotic to combat them. This occurs much urine infections, not finished the treatment because we are better or started to take the antibiotic on our own when we have the first symptoms, leading to the loss of its effectiveness ”, confirms Salazar.

Responsible for pharmacy of USP La Esperanza believes that we are becoming more aware of this problem, however, an effective health education in the responsible use of medicines is very necessary.

Wise antibiotic use

With the objective of raising public awareness of the importance of making good use of antibiotics and raise awareness of the dangers to public health posed by the resistance to antibiotics, the European Parliament adopted declare each November 18 the European day for the prudent use of antibiotics.

So that the population begins to raise awareness about this problem, Salazar considers that there are two lines of work: on the one hand are the measures that follow patients: “ must see a doctor for a diagnosis of the disease, treatment and comply with patterns of treatment according to medical indications. If you have any doubt once begun treatment, call the pharmacist ”.

On the other hand are measures that medical personnel must develop: “ the doctor must explain to the patient what disease suffers from, what will be the treatment, how they should follow him and make sure that the patient has understood ”, said Salazar.

On the USP hospital group

Founded in 1998 and headquartered in Madrid, USP hospital is the first hospital private group in the Spanish market. USP hospital now boasts a network of 12 hospitals and 23 health centres which assist 4,000 doctors that attend each year more than two million patients. In addition, at the beginning of 2012 it is expected that enters into operation a new Hospital USP La Bahía in the Campo de Gibraltar.

The big Auditors may have to split

London (Reuters) – four major auditing firms in the world will have to be split and rename according to a preliminary draft of the European Union (EU) to toughen measures on conflicts of interest and deficiencies which put in evidence the financial crisis.

“The confidence of investors in audits has been hit by the crisis and I believe that the changes are needed in this sector,” said on Wednesday, Michel Barnier, Commissioner of market internal of the EU.

Policy makers questioned the reasons for which the Auditors gave fresh reports many banks which were then rescued by taxpayers when began the crisis financiera.

Barnier said that the recent apparent errors in audits to the banks of AngloIrish and Lehman Brothers, BAE Systems and Olympus, “suggest strongly that audits are not working as they should be”.

Requires a more robust monitoring and “more diversity in which is too concentrated, especially at the high end market”, stated.

Only four Auditors – Ernst & Young, Deloitte, KPMG, and PwC – revised accounts for the majority of the leading companies in the world, a situation which the Commission said was “essentially an oligopoly”.

According to the plan of Barnier, large auditing firms – the four main – will have to separate audit activities which are not linked to this activity, such as management of taxes and other consulting services “to avoid all the risks of conflict of interest”.

Claire Bury, one of the primary responsibility that is under the command of Barnier, said that the plans, if they are approved by the countries of the EU and by the European Parliament, will have impact on the “big four” business models.

The operations of large firms – defined as well as they have income by more than 1.5 billion euros in the EU – which are auditing will need to have structures legally separated.

“, Will have to change the names.” “I guess we will have problems of brand at the end,” said Bury during a press conference

Responsible for the Commission said the sector would need time to adapt, but expected that the new rules are in force in a period of three to five years.

Sex without protection among men enlivens the AIDS epidemic in Latin America

Geneva, 30 nov (EFE).-unprotected sex between men and the lack of national programmes of prevention and treatment aimed at this group are stoking the AIDS epidemic in Latin America, where there are 1.5 million of those infected with the virus HIV.

They are data and conclusions included in the “report on the response Global to HIV/AIDS”, produced by the World Health Organization (who), UNICEF and UNAIDS, and launched today in Geneva.

Against the global trend that has registered a decline of 15% of persons carriers of HIV in the past five years, in Latin America the total number of people living with the virus has risen from 1.3 million in 2001 to 1.5 million in 2010.

What is a seemingly negative data, it is not at all, that the report attributes this increase to the increase in people receiving antiretroviral therapy, which has made reducing the number of people who die from AIDS causes.

In 2010, the number of deaths in the region was 67,000 from a peak of 83,000 from 2001 to 2003.

In the same period there was a drop in the incidence of HIV among children under 15 years: 47,000 carriers in 2001 has passed to 42,000 in 2010; of new infected 6,300 annually has passed to 3,900, and deaths from AIDS-related causes has gone from 4400 to 2,700 per year between 2001 and 2010.

In this context of positive data, the concern of the United Nations is the spread of the virus by the unprotected sex between men, a group which in the last decade the prevalence of the virus has been 10% in 9 of the 14 countries of the region.

Rates of infection in this group reaches 21% in Bolivia, from 19% in Colombia and Uruguay areas, and 12% on average in the 10 cities of Brazil and the three cities of Honduras where carried out case studies on this topic.

According to the report, men who have sex with other men in Latin America are 33% more chances of contracting HIV than the average male.

In a study that was done in 2010 in two provinces of Nicaragua (Managua and Chinandega) it was found that 3% of men who have sex are carriers of HIV, of which only one-third said using condoms.

In another study of 2009, is recalled in the report, has been identified an incidence of the virus in 3.5% among individuals in this group attending Centre public health in Lima.

And one concrete fact: San Pedro Sula (Honduras), found that 9% of men aged 18-24 who have sex are carriers of the AIDS virus.

The problem of transmission not concerned only this group, as highlighted in the report, since many of these men also have sex with women from way to normal.

The authors of this study by the UN denounced the lack of national programmes that are focused on preventing and treating infection of HIV in this social group, being the only country who spent more than 5% of its total investment to prevention activities to programmes for sex between men Peru.

Peru was also the exception among the Andean, together with Central Americans demonstrated lack of resources to address and combat the problem, as said in press conference Gottfried Hirnschall, responsible for the fight against HIV in the who.

Hirnschall noted that these countries need to rethink their goals, and urged them to overcome the stigma that continues to weigh on homosexual relationships with campaigns against homophobia, which have already yielded positive results in Mexico, Brazil and Argentina.

The head of the who said that the stigma lies also with transsexuals, a collective on which there is little information or unreliable information, in the incidence of HIV.

The report cites a study conducted in 13 cities of Argentina, who revealed “alarming” the HIV prevalence rates, and that 34% of transgender prostitutes are carriers of the virus.
