Shorter treatment helps some patients with hepatitis C

New YORK (Reuters Health) – manage Peginterferon and ribavirin for 24 weeks after completing treatment combined with telaprevir is as effective as the regime of 48 weeks to cure chronic infection by the virus of hepatitis C (HCV), according to a new Studio.

Reduce the duration of treatment reduces the risk of adverse effects, but only in patients who respond quickly to the initial phase of treatment, publishes a team in the New England Journal of Medicine internet Edition.

“In our study, two-thirds (65 percent) patients were candidates to receive shorter therapy due to its extended rapid viral response.” “Were they could have indicated Peginterferon and ribavirin for 24 weeks”, said the lead author, Dr. Kenneth Sherman, of the school of Medicine of the University of Cincinnati.

The study of non-inferiority was designed to confirm that patients with the fourth and twelfth weeks not detectable HCV RNA evolved not worse with an abbreviated system of therapy with Peginterferon and Ribavirin, combined in a first stage with telaprevir.

Vertex Pharmaceutical

and Tibotec funded study ILLUMINATE. The administration of food and medicines of United States (FDA for its acronym in English) approved telaprevir may last.

The study began with the selection of volunteers from Belgium, the Netherlands and the United States genotype 1 HCV carriers. None had been treated prior.

A 540 was told to take 12 weeks of treatment with telaprevir (750 mg every eight hours), Peginterferon alfa-2a (180 mcg per week) and Ribavirin (1,000-1,200 mg per day), most combination of Peginterferon and Ribavirin during other 12 weeks.

One hundred patients left the initial phase of treatment (including 12 which made him by virological failure, 62 by adverse effects and 12 because they refused to continue the treatment or withdrawn consent).

Of 440 volunteers who continued in the study, 322 were extended rapid virological response: HCV RNA kept undetectable in the blood at 20 weeks. Randomly, stated them stopping treatment to reach 24 weeks or continue it another 24 weeks.

92 Percent of the 162 with extended rapid virological response ending treatment at 24 weeks was still free of virus to 48 weeks.

The 88 percent of the 160 remaining, who continued therapy with Peginterferon and ribavirin for another 24 weeks, remained a virological response sustained at 48 weeks. In this group, 41 left 24 weeks of treatment.

On the other hand, 64 percent of 118 patients without rapid response with other 24 weeks of treatment, had a sustained virological response. Thirty-nine of them suspended treatment.

Total, 72 percent responded quickly to the regime of three drugs to the fourth week. 12 Weeks, 65 per cent remained traces of the virus in the body

In the essay ILLUMINATE, 18 per cent of volunteers treatment was withdrawn by the adverse effects: anaemia by 39 per cent of the patients (by 6 per cent of the cases was serious and 32 participants needed transfusions) and rash in 37 percent of patients (5 per cent was serious).

Most of the cases of rash appeared in the first eight weeks of treatment.

Serious adverse effects were more common in the group treated for 48 weeks with Peginterferon and Ribavirin. (The rate was 10 percent, compared with 2 percent of patients with limited treatment).

Fatigue was the most common secondary effect (68 percent), followed by pruritus (51 percent), nausea (47 percent), anemia (39 percent), headaches (38 percent), insomnia (32 per cent), diarrhoea (28 percent) and disease type influenza (26 percent), it was reported the team.

The traditional 48 weeks of Peginterferon and Ribavirin therapy costs about $30,000, said Sherman. But the telaprevir is more expensive. “I saw figures of $49,000,” indicated.

“This is a lot of money for a short treatment, but must be compared with the costs to society of hepatitis C without trying, in which even a liver transplant could cost hundreds of thousands of dollars, not to mention previous transplant associated medical costs,” finished Sherman.

Malaria vaccine provides little protection: study

New YORK (Reuters Health) – an experimental vaccine against malaria proved to be not effective in field trials, except when the participants were infected with a specific strain of the parasite that causes the disease.

The study carried out in Mali revealed 48.4 percent of cases of malaria among 199 African children vaccinated and a 54.4 per cent of cases in 201 children immunized against rabies as a placebo (control group). The difference was not statistically significant.

But Dr Christopher Plowe, of the school of Medicine of the University of Maryland, in Baltimore, found that the results published in the New England Journal of Medicine, internet Edition are encouraging because the vaccine should attack the parasite in the blood, the stadium in which appear the symptoms.

“En.” the first time that a blood-stage vaccine offers some level of protection against clinical malaria “Attenuates the stadium that causes illness and death”, expressed.

Plowe explained that the species of the parasite against which the vaccine proved most promising represents between 3 and 12 per cent of cases of malaria.

In addition to this vaccine, developed at the Institute of research of the Army Walter Reed in United States, and known as FMP2.1/AS02 (A), there are other 17 vaccines against malaria are being tested, according to the World Health Organization (who).

The who estimates that there are 247 million cases of malaria in the world and 718.000 deaths per year.

Malaria is the leading cause of death and disease in Mali. The team assessed 745 children aged between 1 and 6. 400 Participants received at least one intramuscular doses (at 367 them were three doses). The injections were applied with monthly intervals and 383 children is them he controlled six months more in the season of malaria.

“Vaccine not provided a high level of protection against clinical malaria, but according to secondary outcomes would be effective for a species of parasite,” advises the team. Not attributed deaths or adverse effects serious.

In a previous trial, the vaccine, for the clone 3D7 of the species Plasmodium falciparum, had elevated levels of antibodies in blood

The National Institute of Allergy and infectious diseases, national institutes of health and the United States Defense Department funded the study.

Warn to travellers after report of 7 cases of measles in Colombian Caribbean

Bogota, 16 sep (EFE).-the Ministry of Social protection of Colombia today launched a call to alert travelers to visit the Atlantic coast, in the North of the country, after the official of the health of seven confirmed of measles cases authorities report

Is of “a call for all travellers to go to move to the Atlantic coast to 15 days prior to travel, to attend institutions (..)” “and were vaccinated against measles and rubella, for free,” warns today a recommendation of this portfolio in Bogotá

Travellers “should be alert to symptoms such as fever, rash all over his body, cough, stuffy nose and eye irritation”, adds warning.

In the last hours of the National Institute of health (INS) confirmed a new case of measles, which are already 7 reported to date.

The latter affects a child under 8 months of age, in the city of Barranquilla.

Warnings were made by the Colombian Government to host the Pan American Health Organization (PAHO) recommendations.

The Pan-American body invites travellers to be vaccinated against measles and rubella to reduce the risk of reintroduction of the virus of these diseases already eliminated in America.

In recent weeks some 200,000 people have been vaccinated in areas of the Caribbean Colombian coast, of which about 80,000 in Barranquilla, city in which occurs the resurgence of the measles.

Colombian health authorities recommended, Furthermore, that the personnel of health, transport, and tourism sectors are also vaccinated against diseases viral.

In Colombia not had cases of measles since 2002, he recalled today the Ministry of the protection Social.

Health officials in the Department of Cauca Department (southwest), on the other hand, revealed that Friday in a rural area near Popayan, capital of that territory, there is an outbreak of chicken pox with three tens of cases. EFE

Allergy to cows milk causes problems of ear, nose, throat

New YORK (Reuters Health) – the constant ear problems, nose and throat in infants and young children may be associated with food allergies, particularly to the protein of cow’s milk, according to a study presented at the annual meeting of the Academy American of Otolaringología, in San Francisco.

Diseases such as otitis media refractory or oropharyngeal dysphagia usually improve if children removed certain foods, commented Dr. Angela Paddack, of the school of Medicine of the University of Arkansas in Little Rock.

“Hypersensitivity to food is a common diagnosis.” “These children have gastrointestinal disorders, but a subgroup has ear, nose and throat problems and do not improve with the usual therapy for reflux, said Paddack.

“Improve when retire them to determine if they are allergic to cow’s milk protein foods”, added.

With his team, the researcher reviewed information from 101 children who might be allergic to protein and which had been treated at clinics in Gastroenterology and otolaringología.

Children had symptoms such as diarrhea, vomiting, reflux and constipation. When them was on a diet to eliminate cow’s milk, 91 percent improved.

Twenty-five of those children had problems of ear, nose, and throat that had not responded to therapy for reflux. The most common were higher in 72 per cent dysphagia oropharynx in 80 percent of cases and continuous airway congestion

The author pointed out that 60 percent of children with problems of ear, nose and throat improved the diet of elimination.

“Cow’s milk allergy can cause the same reaction in the Airways higher than in the gut.” These children develop a chronic inflammatory response. One of the main symptoms was chronic congestion, often demanded the use of assisted ventilation, pointed out.

In addition, “had difficulty swallowing, which caused many problems with food.” “Parents should know that if they have a child with these symptoms that do not improve with the traditional treatment should think about allergy to protein from the milk of cow as a probable cause,” said Paddack.

A new strain of a virus affects dogs in the Argentina

(www.neomundo.com.ar_–_Agencia_CyTA_–_Instituto_Leloir). A study published in the scientific journal Virus Research dongles on a high presence in the Argentina of a variant of the canine Parvovirus that causes a disease in puppies of up to approximately 6 months of age, causing gastroenteritis haemorrhagic.

The authors of the study – the Animal Virology, Institute of science and technology centre “Dr. Cesar Milstein” of CONICET and the Foundation studies in Animal Virology – analysed 79 stool samples of which 55 were infected with the virus.

“In 91% of the samples detected virus corresponded to ECA CPV2c.” Parvovirus evolves continually; the first strain, described in the Decade of the 1970s, called CPV-2 (currently present in commercial vaccines). Later appeared chronologically CPV-2b and CPV-2a variants in the 1980s. The latest variant (CPV-2 c), described for the first time in Italy in 2001, it was detected in our country, after an outbreak in Bahia Blanca in 2009, detecting then in the rest of the country. However, subsequent analysis of samples showed that that alternative had been present in Argentina, from the 2003 “, said Dr. Marina Rooster Calderón, CONICET researcher who carried out the study under the direction of Dr. José La Torre, director of the Institute of science and technology César Milstein.

ATYPICAL table

The difference in clinical level between ECA CPV2c (identified by the authors of the study) and other variants, is that it can produce an atypical clinical picture which includes not bloody discharge diarrhoea and resembles a liver disease or kidney.

“These clinical manifestations can mislead the veterinary clinical, making difficult their diagnosis”, emphasized the researchers. And added that the molecular diagnosis (PCR) is important to rule out this serious infection.

“Atypical symptoms and the fact that the difference between CPV-2b and CPV-2 c is only an amino acid (an organic molecule), make very difficult their identification by the classical procedures.” It is therefore required the application of molecular techniques such as PCR and sequencing of DNA for identification, stressed Dr. La Torre.

Also the researchers noted that current vaccines not preparing the organism of the dogs to defend effectively ECA CPV2c.

“This is a new variant of the virus that can even also affect Cubs who received the vaccine, and particularly those who did not complete the recommended immunization schedule (3-4 initial doses, with annual revaccination)”, explained Dr. Gallo Calderón.

New VACCINES

In this sense, La Torre said that it is advisable to produce new vaccines that are effective against all strains of parvovirus until now identified, including ECA CPV2c.

And continued: “our group is on board with a national company in the development of up-to-date vaccine.” This is not easy because the Parvovirus vaccines are based on attenuated strains, i.e., are modified live virus to immunize but do not produce clinical disease. To develop this type of vaccine, it takes many years. Therefore, the only quick way is the development of recombinant vaccines containing sequences of amino acids corresponding to the new variants. “Added this to the development of new adjuvants and new schemes of immunization aimed to generate levels of protection in vaccinated animals.”

Dogs affected by parvovirus treatment is rehydration parenteral.

“Is providing intravenously electrolytes to replace losses due to severe diarrhea and the supply of antibiotics and prevent secondary infections”, explained Dr. Gallo Calderón.

Prevention and treatment

Due to his extreme infectivity, affected animals must be isolated, performing a thorough disinfection of the site, in order to avoid malware, dispersal, in places where a greater number of animals, such as veterinary and breeding is concentrated, say the authors of the study. And clarify that, despite the fact that vaccines are not completely effective for ECA CPV2c, it is advisable to vaccinate puppies with regular vaccination scheme recommended by the veterinary trials.

Vaccines are important, but not enough tools emphasizes Dr. Gallo Calderón. And concludes: “To prevent disease, we recommend making vaccination in time and manner, as recommended by the international protocols”. It is also necessary to carry out a cleaning and disinfection of areas where there were infected animals; not bring animals to establishments in which could be infection; ask the vet you come to the home to carry out the vaccination; “and all measures hygienic – sanitary recommended by our veterinary or health authorities as for example, the Elimination of fecal matter in public.”
