Hospital das Clínicas deploys dental service in Icu

Dental team visit daily inpatients in 115 beds of Central Institute to identify and remove outbreaks of oral infection in patients.

Hospital

a little more than six months, patients in Icu of 115 beds of nine Central Institute of the Hospital das Clínicas are visited daily by dentists who aim to identify and address potential sources of infection in the mouth of the

patients.

the mouth is one of the ports of entry for respiratory infections, mainly. If you are not clean, facilitates the proliferation of bacteria. If not treated, they can compromise the patient’s clinical picture, especially of tubed and delay the process of high

.

Before, the dental service in ICU was triggered only under medical demand. In addition, already was part of routine dental treatment of elective patients – before the scheduled surgery to prevent infections. –

Now, a hygiene Protocol was created: the nurses make teeth cleaning – which includes scraping and brushing the tongue – and dentists seek sources of infection

.

If a patient is with plaque, for example, the team makes the scraping and hygiene at the time. If the tooth is ignited at the point of degrading treatment, it is extracted – two weeks ago the tooth of a patient with tetanus was extracted because it could be the gateway of

infection.

“ hospital dentistry’s goal is to treat acute outbreaks and opportunistic, as pus in the gums, plaque, periodontitis. We will not restore caries, because it is chronic infection, acute not ”, says Maria Paula Siqueira de Melo Peres, Director of the Division of hospital Dentistry.

and it’s not just in ICU of HC that dentists are present on a daily basis. For eight years the service was implemented in the Santa Casa de Barretos by Teresa Márcia Morais, Chair of the Department of Dentistry of Brazilian Medicine Association (Amib).

“ we see fewer cases of infection, less use of medicines and high faster. Is an economy and not cost more. ” Tereza account that one of the most striking was that of a patient hospitalized had 37 days because of an infection in his heart. The case has not improved and the patient went to ICU. Dentists have concluded that the focus of infection was the mouth. “ He had a severe periodontal disease. We removed the foci and draw two teeth. He stayed a week in ICU and today is well. “

Within the framework of the 16th national course of Alzheimer’s disease, held in Palma de Mallorca in February 2011, met a group of experts in Alzheimer’s disease and other dementias to establish an interdisciplinary dialogue. The discussion addressed the current situation of dementia care, with special mention of the possible ways of organizing it to patients and their families. The proposed starting point was the discussion on the necessity of units of dementia, its structure and its objectives. The conclusions of this meeting can be summarized in the following:

considering:

-the prevalence of dementia is very high and has characteristic epidemic.

-It is important to make early diagnosis of the disease.

-That the spectrum of clinical manifestations of dementia is broad and makes care needs change according to the stadium and the type of dementia.

-Currently not be available treatment that can modify the inexorable trend towards severe dementia.

– Cognitive impairment and dementia generate a high demand for direct health care in the areas of primary care, specialized care and resources partner health.

-That it is currently the family who bears the greater burden in caring for the sick.

-The classic care model of evaluation in primary and specialized care, does not meet the needs of the patient and his family.

-the need to organize assistance to cognitive impairment and dementia through the training of units interdisciplinary.

-That these units must have equipment diagnosis and comprehensive follow-up of the patient and his family-oriented. They shall also include professionals in primary care, Neurology, Neuropsychology, geriatrics, psychiatry, Nuclear Neuroradiology-medicine, rehabilitation, nursing and Social work.

-That the objectives of these units must be:

-precise identification of the clínica-biológica situation – social ill.

-the diagnosis of disease or diseases responsible for.

-establish a therapeutic plan that includes pharmacological treatment, non-pharmacological and social intervention relevant.

-establish a plan of monitoring that allows to adjust the therapeutic plan developments observed.

-evaluate the burden of non-professional caregiver of the patient and establish a plan of support.

-assess the level of knowledge they have relatives of this pathology to be able to form the family in dementia, as well as offer training focused on concrete and specific skills enabling them to deal with competition, to the difficulties cognitive, emotional, behavioural and social that they can appear during the process.

-That the assistance should include all stages of the disease, cognitive impairment without dementia dementia until serious.

-That these cognitive impairment and dementia units allow the optimization of the resources available and the incorporation of new tools for diagnosis and treatment.

-Which is important to the functional concept of the unit rather than structural. It is necessary dementia units forming part of a health care network to dementia.

-Which should promote a change in the attitude of society from Alzheimer’s disease and other dementias, oriented to change current, by the commitment and the accompaniment of the patient and his family. fear and rejection

Therefore

More than 8,000 people suffer from some sort of hereditary ataxia in Spain, according to data from the Spanish Neurology society.

More than 8,000 people suffer from some sort of hereditary ataxia in Spain, according to data of the society Spanish of Neurology.

, September 25: day international of the Ataxia.

-Because the number of patients with ataxia represents a small percentage of the population, they are fairly unknown society diseases. The Sociedad Española de Neurología (SEN) would like to take this date to provide a social recognition for these diseases to boost research and development of effective treatments

Spain, 2011-September next September 25 commemorates the international day of Ataxia, a date which aims to promote social awareness to the heredodegenerativas Lazo. Although currently there is no official statistics, the Sociedad Española de Neurología (SEN) estimates about 8,000 people suffer from some sort of hereditary ataxia in Spain. Its relative low impact makes such diseases are nearby within the Group of rare diseases and, therefore, at present, there a deep social ignorance of them, which conditions are not allocate sufficient resources for their research and development of new treatments.

Under the term Lazo heredodegenerativas encompass over 40 types of degenerative diseases of the nervous system characterized by loss of balance and coordination in the implementation of voluntary movements. Generally are gradual, evolutionary, diseases highly disabling for which there is no curative treatment.

Usually, the hereditary ataxias are usually classified according to the way in which they are inherited: recessive autosomal (which include the spinocerebellar, also called SCA Lazo), autosomal dominant (as Friedrich’s Ataxia), ataxias associated with mitochondrial dysfunction and the linked to chromosome X.

Due to the wide variety of ataxia heredodegenerativas referred to within this term, the symptoms of these diseases is very varied, however, the lack of coordination of movements, which can affect any part of the body, is common to all of them feature. This lack of coordination can manifest itself in the lack of balance, a lack of coordination of the upper limbs (limb loss) or the deterioration in the control of eye movements or the muscles involved in speech. “ Although some types of ataxia are more severe than others, it is important to note that within a single type, not all patients will have the same severity of disease. In some cases the symptoms are mild and with a slower progression, while in others the disease may be more aggressive and lead to an early functional inability. But the truth is that they can start with a lack of balance small and end up being highly disabling ”, drew the DRA. Rosario Luquín Piudo, Coordinator of the Group of study of disorders of the movement of the SEN.

The Lazo heredodegenerativas tend to develop more often during childhood and adolescence, and as it happens with many degenerative diseases of the nervous system, there is currently a definitive curative treatment for these entities. However, “ many of the symptoms and complications associated with these diseases can be treated. In this way, although they are incurable diseases, that is can help patients to have a good quality of life as long as possible ”, ensures the DRA. Rosario Luquín Piudo.

“ Heredodegenerativas ataxias are very cruel diseases and, unfortunately, very neglected both by society and by the scientific community. For that reason it is so important that, for more than ten years, will commemorate this date. September 25, allows that, at least for one day, these diseases and their affected charge the visibility they deserve ”, concludes the DRA. Rosario Luquín Piudo.
