Madrid, 2011-August the consultation offers specific nursing care to patients who are treated in outpatient medical treatment requires the administration of medications via parenteral in the home of the patient. It is part of ambulatory dispensation of drugs unit and offers a new model of care to the patient.

Initially these cases were cared for in Hospital of the Centre, and this new consultation, patients have come to be served in the pharmacy service. Completed the care of the patient by the doctor specialist, it goes to the pharmacy service, where in addition to receiving pharmaceutical care linked to the process of dispensing of the medicinal product, is given attention and specialized nursing care for the administration of his treatment at home.

The consultation attends to patients with digestive pathology (liver diseases and inflammatory bowel disease) through consultations of high resolution and the patients of Neurology (MS) that need a self-administration of their parenteral medication at home.

It currently serves more than 200 patients. It is expected to be extended this consultation to other outpatient delivery programmes, that 70% of patients treated in ambulatory dispensing unit need a medication administered parenterally.

After consensus among pharmacy service and the management of nursing, was opened in the hospital an independent physical space to provide the patient a greater degree of privacy in the consultation. Work is coordinated with the dispensation ambulatory unit and has continued medical care to solve possible emergency situations.

With the addition of this new provision, the pharmacy service modifies the model of patient care, betting on a care comprehensive farmacoterapéutica whose objective is to achieve greater efficiency by ensuring the maximum well-being for the patient.
