Clinical laboratory specialists can diagnose and classify urological and nephrologic diseases studying the red cells excreted in the urine

Madrid, October of 2011- the study of urinary sediment is a source of important information in the evaluation of kidney injury and more when around 20% of the general population suffers conditions in the apparatus urinal. from my experience, in an only Institute of Urology, Nephrology and Andrology, dedicated to deal with this kind of problem, carried more than 60,000 annual outpatient visits and nearly 4,000 surgical ”, says Dr. Fernando Dalet, Member of the Spanish society of Clinical Biochemistry and Molecular Pathology (SEQC).

As in other pathologies, the early diagnosis of renal lesions is essential, and in doing so, they are vital tests to achieve this. In this sense, the Spanish society of Clinical Biochemistry and Molecular Pathology (SEQC) aims to put on the table the important role of laboratory tests to determine the majority of conditions associated with the urinary system. To this end, has organised the distinction of the red cells of glomerular origin seminar: an illusion or a useful tool for clinical diagnosis?, sponsored by the Fundación José Luis Castaño for the development of Clinical Biochemistry.

Urine sediment is, in practice, the last of the manual analysis with subjective final evaluation, which means, in short, the more you know and greater experience has the best researcher and more specialized results are obtained ”, explains Dr. Dalet. hence, the importance of generating knowledge and report on developments in this field that, as with everything, there are easy cases of rapid diagnosis and other complex ”, adds the expert.

Today, have different tests to determine if a person suffers or not kidney disease. There are diagnostic tests for the image (x-rays, ultrasound and CT), the Liveview (Endoscopy) and the analytical. Almost all, except ultrasonography and urine are invasive tests, i.e., is undermining the integrity of the patient and there is always some risk ”, reveals Dr. Dalet. This reason and others such as security, its easy to obtain and its very low cost lead experts to recommend this diagnostic test.

The red cells as clinical diagnostic tool

Hematuria (blood in the urine loss) is a sign (not a symptom) which immediately alerts not only to the clinic but also to the patient, which attends the doctor’s presuroso. The main reason for this fact is that all urological pathologies and almost all the nephrologic show this sign ”, explains Dr. Dalet.

Currently, the clinical laboratory has a method that investigates the morphological abnormalities of the red cells and depending on the result classifies hematuria in isomórfica or Dimorphous. Hematuria isomórfica comes from the direct discharge of blood capillaries to the urinary stream, as occurs in urological diseases, so the patient can lead to the urologist ”, says Dr. Dalet. On the other hand, adds expert, Dimorphous hematuria comes the filtration unit nefrónica and indicates a lesion of the glomerulus. This means that this type of hematurias will be present exclusively in own kidney diseases of nefrológico origin, or in those of a systemic nature with impact on the kidney as hypertension, diabetes, gout or dislipemias, among others ”.

In this regard and in conclusion, Dr. Dalet says that thanks to a simple test of urine, which has a cost very low for the national health system, can control and keep track of the number of people over age 50 has one or more of the above diseases. This is a challenge and a change in attitude that still continue costly analysis of blood as a follow-up of these patients ”.
