Surgery of liver metastases from colorectal cancer: the best guarantee of cure for patient.

-el Hospital de Torrevieja address multidisciplinary way treating patients with liver Metastases of cancer colorectal (MHCCR), which is a major challenge for public assistance with around 1.2 million annual cases around the world

– is estimated that one third of patients with liver metastases from colorectal origin can survive beyond the 10 years. Therefore, the MHCCR surgery is the only treatment that offers the possibility of healing

Torrevieja, November of 2011- the unit of hepatic surgery in the Hospital in Torrevieja, to address multidisciplinary way treating patients with liver Metastases of cancer colorectal (MHCCR)which is a major challenge for public assistance with around 1.2 million annual cases around the world.

At the Hospital in Torrevieja, patients with colorectal cancer are treated by a multidisciplinary team of oncologists, surgeons, anaesthetists, radiologists, pathologists and hepatólogos. Hepatic surgery unit is composed by surgeons with extensive experience in the management of this type of patient, which leads to excellent results both in terms of morbidity and mortality and long-term disease good prognosis.

It is estimated that one third of patients with liver metastases from colorectal origin can survive beyond age 10. Therefore, the MHCCR surgery is the only treatment that offers the possibility of healing and has been shown to provide clear benefits in the survival of these patients.

The liver is the most common development of MHCCR, observed in more than 25% of patients at the time of diagnosis and by up to 50-75% in the evolutionary development of the disease (in the 3 years after the diagnosis of the primary tumor). Despite the aggressiveness of the cancer, the torrevejense Center professionals argue that surgery of liver metastases offers the possibility of healing in selected patients and is recognized as the treatment of choice in this type of sick.

The median survival of patients with cancer colorectal metastatic treated with support measures is of approximately 6 months. The current treatment with chemotherapy has increased this median survival to about 20 months, liver surgery with curative intent in these patients can get survivals of over 40% at 5 years.

Liver surgery has progressed in parallel with the improvements occurred in chemotherapy, and radiology. Progress in surgical techniques has much reduced adverse effects, as evidenced by the latest series of patients undergoing resection of liver metastases with a mortality rate that ranges from 0 to 3% and less than 30 per cent morbidity.

The experience of the Hospital has a major impact on the outcome: the morbidity and mortality in liver resections decreases due to reverse to the number of cases carried out in the Center. In this sense, it is very important that the patient is treated in a hospital with a high volume of annual liver surgery, as well in the Hospital in Torrevieja.

The DRA. Amador, head of the service of General Surgery and digestive apparatus of the Hospital in Torrevieja thinks that “ surgical advances, progress in liver surgery and radiology and chemotherapy improvements made to increase the number of patients candidates for hepatic resection and that such treatment is carried out in hospitals where there was no one classical tradition of liver surgery as such ”. Furthermore, believes that “ is important that these centres be subject to audits and join international registrations where can guarantee that what you are doing is doing well, not only in terms of morbidity and mortality but also, and of course, in terms of survival ”.

Careful patient selection

Only between 10% and 20% of patients with MHCCR they initially meet the requirements for surgical resection. During the last decade, significant advances have taken place in the management of the MHCCR, especially in three areas: Oncology, radiology and surgery. These advances have increased the number of patients that may be offered surgery by up to 20-30% of cases, with a survival at 5 years from 35-50%.

The Neoadyuvante chemotherapy, which is performed before surgery, is recognized as an effective treatment to make patients suffering liver metastases that initially are not candidates for surgery, they may be, provided that they submit a response positive treatment.

-AstraZeneca and ACS presented at the XVIII national meeting of the AEC (November 9 in Pamplona) Guide in colorectal surgery.

-la Réunion in Pamplona is a date for the meeting of professionals in surgery in our country

Pamplona, November 2011- AstraZeneca and ACS (Spanish Association of Surgeons) presented the second edition of the guide in colorectal surgery as part of the 18th National meeting of the AEC. This new edition comes ten years after the publication of the first and updated knowledge in the field of colorectal surgery after an important development in recent years.

The Guide, which is aimed mainly at colorectal surgeons and residents, is also a manual of knowledge to all professionals who wish to be aware of developments in colorectal surgery.

The Guide responds to the need for a re-release which updated treatments, diagnoses or procedures which were obsolete and that it was necessary to adapt. In the words of the Coordinator of the new guide and former President of the SRA, Dr. Héctor Ortiz: “ guide was needed, especially after the experienced progress, including: the treatment of diseases such as Crohn’s or ulcerative colitis and colorectal cancer, as well as the diagnostic or surgical technique ”.

Specialists who have collaborated in this guide are first level colorectal surgery professionals and have multiple previous publications in many topics addressed.

The incorporation of contents that highlight the importance of multidisciplinary teams composed by pathologists, oncologists, surgeons and nurses is one of the most prominent in colorectal surgery changes. In our country, the multidisciplinary teams are already a fact and there are many hospitals that already have them to treat their patients.

Nephrologists and renal patients propose actions to improve the sustainability of dialysis treatment.

Nephrologists and renal patients propose actions to improve the sustainability of treatment with dialysis.

quality and sustainability of the TSR conclusions from the Forum II.

-correct planning of treatment, ensure the Nephrologists in home peritoneal dialysis training, ensuring equity in access to all treatments and imitate the model of empowerment of transplants, some of the proposals of the GADDPE

-the home peritoneal dialysis improves the quality of life of the patientoffers clinical benefits, and also cheaper than hemodialysis for healthcare

Madrid, November 2011- the support group for the development of the Peritoneal Dialysis in Spain (GADDPE) and the Fundación Renal ALCER presented document “ conclusions of the II Forum of quality and sustainability of the Renal Sustitutivo treatment. Recommendations and proposals ” that gathers the main exhibitions held in II Forum of quality and sustainability of the TSR. The meeting brought together patients, members of the health administration and the Nephrology professionals and noted the actions undertaken to improve renal replacement therapy (TSR) in Spain.

The Forum was revealed that in the current economic context, the optimization of resources and the sustainability of the public system is key. In the case of Chronic Renal disease, in which only 50,000 patients consume nearly 3% of the health budget, it is necessary to pay special attention to the efficiency of the treatment. Taking into account that increase the percentage of patients in home peritoneal dialysis treatment represents a saving of 500 million euros to the SNS in 15 years, was in evidence the need for increasing the use of this technique.

In addition to an overview of the current situation of chronic renal failure and its treatment in Spain, the document contains proposals and regional experiences which were discussed in the Forum which includes the strategy of Chronic Renal disease of Balearic Islands, and the actions carried out in Castile and León, SpainComunidad Valenciana, Castile – La Mancha and Andalucia.

Proposals to improve the renal replacement therapy

Finally, the document includes the proposals for the GADDPE to improve the renal replacement therapy, which include:

-correct planning of the TSR,

-promote the entailment of the patients in the process of shared decision-making,

-mimic the Spanish model of transplants in the empowerment of the home peritoneal dialysiswhich means planning, provide resources and promote a model reverts in satisfaction, leadership, economic efficiency and social prestige, but above all get results in health and quality of life,

-ensure the formation of the Nephrologists in home peritoneal dialysis,

-ensuring equity in access to all available treatmentsensuring that patients receive adequate information on informed consent.

Home Peritoneal Dialysis

Peritoneal dialysis is a form of renal replacement therapy home used in patients with chronic renal failure. The Elimination of toxic substances is carried out through the peritoneal membrane and can be carried out manually or automated, at night, while sleeping. The only patient attends hospital to review each one or two months. This helps the patient continue with their work and personal life skills and gives you more freedom and privacy in their treatment.

Peritoneal dialysis presents:

-Social benefits, as it allows the patient to keep your work and personal life and provides you with greater independence, intimacy, autonomy and quality of life;

-advantages medical, because it is considered an excellent beginning of dialysis modality, is very suitable for patients who are on waiting list to receive a transplant, is recommended for all ages and, for its continuous nature, is the closest to the normal functioning of the kidneys.

-benefits for the national system of health, that its use would result in a saving of EUR 500 million in 15 years. According to an economic evaluation, the annual average cost per patient of treatment patients 46,659 euros in hemodialysis and 32.432 euros in home DP, i.e. hemodialysis is 44 per cent more expensive than the DP.

About the GADDPE

The support group for the development of Peritoneal Dialysis (GADDPE) aims to raise the health administration and society in general on the problem of under-utilization of peritoneal dialysis on Spain. The group consists of members of the Group of dialysis Peritoneal the society Spanish of Nephrology (SEN), the Spanish society of nursing Nefrológica (SEDEN), Fundación Renal Alcer Spain (FRAE), the Kidney Foundation Iñigo Alvarez de Toledo (FRIAT) and professors from different universities of medicine and nursing.

It is estimated that more than 1.5 million Spaniards suffers from Diabetes Mellitus or still does not know.

It is estimated that more than 1.5 million Spaniards suffers from Diabetes Mellitus or not yet known.

14 November, day world of Diabetes.

-Clinical London held a day of open door of their clinics offering free of charge, to anyone want you, an analytics to detect when

Spain, November 2011- 8.1 per cent of the age of 18 Spanish suffers from Diabetes Mellitus, which is more than three million people affected, according to the study di@betes.es. But what is even more striking and worrying is that there is another 3.9% of the population who still does not know that it suffers from this disease, a total of 3.111.641 people.

“ La Diabetes Mellitus is a metabolic disorder that manifests itself by glucose in the blood (blood sugar) levels beyond the normal limits. The cause is an anomaly in the production or the functioning of insulin by the pancreas. If it is not properly, these levels reach too high values, giving rise to acute or chronic complications of Diabetes ” explains Elena de la Fuente, clinical nutritionist, London.

When there is no insulin (Diabetes Mellitus type I) or the production system is not working properly (Diabetes Mellitus type II), the sugar does not pass blood to vital organs and its function is poor. Some symptoms that can lead to suspicion of a Diabetes undiagnosed are the polyphagia, polydipsia and polyuria. Found that those affected and who still have no diagnosis “ they eat more food because glucose does not fit well into the cell and more hungry (polyphagia), drink plenty of water because their blood glucose concentration is high and this results in thirst (polydipsia) and urinate much by the large amount of liquid ingested (polyuria) ” explains Dr. Eduardo Morejón, head of the unit of obesity and nutrition clinic London.

Cardiovascular Risk

A sick untreated runs a significant risk of some form of cardiovascular disease as well as having a “ systemic vascular impairment causing lesions in multiple tissues, with particularly significant harm in the small vessels of the retina, kidneys and the peripheral nerves – continues Dr. Morejón-. This is one of the leading causes of blindness, amputations and terminal kidney disease ”.

Weight loss is a significant health improvement

In this sense clinic London specialists explain that the onset of Diabetes tends to be associated with overweight and obesity, so monitor weight and follow appropriate nutritional guidelines will make you lose weight but also it will positively affect health. For example, a study carried out among more than 600 patients who have lost weight made multidisciplinary nutritional programmes confirms that they also have significantly improved their health. Thus 85% improve or cure your blood sugar levels, more than 90% improve his high blood pressure or cease to be hypertensive, and 80% removes their respiratory problems such as the sleep apnea

For all these reasons clinic London yesterday offered free analytics all over 18 who came to their clinics.

Clinic London has 13 centres in Spain, more than 10 years of experience and belongs to the Bavaria group. It was the first clinic European in getting the certificate of quality ISO 9001-2000.

-the latest ratings emphasize the need to know the genetic and molecular alterations in order to better establish the prognosis and treatment of Myelodysplastic syndromes

Madrid, November 2011- La Sociedad Española de Hematología Hemoterapia (AEHH), has among its purposes the scientific advancement in relation to the study and treatment of diseases caused by alterations in blood components. Why, it awards grants to projects of basic research, to advance in the knowledge of the meaning of the alterations and to identify factors of prognostic value through his Foundation.

At the Congress the AEHH LIII, recently held in Zaragoza, were awarded five new fellowships for research. “ proposed items relate primarily to hematologic cancers with poor prognosis and are aimed at studying genetic or molecular ”, points out Dr. Carmen Burgaleta, President of the AEHH. “ With the award of these fellowships aim to stimulate interest in research among the younger specialists ”, add the hematóloga.

The main reason for study scholarships are the myelodysplastic syndromes (MDS). This pathology includes a complex group of hemopathies stemmed from the bone marrow, which is characterized by an alteration in the maturation of the elements of the blood, who studied with Cytopenias of varying intensity that can evolve into acute leukemia or maintain a chronic course.

The SMD appear approximately 7 of every 100,000 inhabitants and have higher incidence in people over 60 years, since its presence increases progressively with age. According to Dr. Burgaleta, “ latest ratings emphasize the need to know the alterations genetic and molecular in order to better establish his prognosis and adapt accordingly, treatment ”. The availability of new therapeutic agents to modify its evolution has revolutionized research in these processes.

The scholarships have been granted to the following research projects:

– “ is the study of the genetic changes of patients with myelodysplastic syndrome 5q-treated with lenalidomide ”.

-“ The search for patterns of response in relation to the study of Hematopoiesis by flow cytometry in affected patients of myelodysplastic syndrome of high-risk treatment with 5-Azacitidine ”

– “ study of trisomy 8 in patients with myelodysplastic syndrome and/or Behçet syndrome ”.

The projects will be carried out in the service of Hematology of the Hospital del Mar, in Barcelona, and in the service of Hematology and Salamanca cancer research centre.

Likewise, the foundation of the AEHH has awarded several scholarships to leukaemia research projects:

– “ study of the frequency and significance prognosis of deletions/mutations of the gene IKZF1 and the State of hypermethylation of genes hsa-miR-375, HDPR1 (DACT1), SHP1 (PTPN6), P16 (CDKN2A), hsa-miR-9-3 and DBC1 in adult patients with acute lymphoblastic leukemia without reordering BCR-ABL1 included in treatment protocols adapted to the risk of the Group PETHEMA (programme Spanish of therapies in hematology.)”, of the Hospital Germans Trias i Pujol, Badalona (Barcelona).

-Project about disease Graft versus host in transplanted patients: “ study of the usefulness of the Zalypsis in disease prevention Graft versus host after allogeneic hematopoietic precursors transplant ”, from Hospital Clínico Universitario de Salamanca.

On the Spanish society of Haematology and haemotherapy (AEHH)

The AEHH is a scientific society whose purpose is the promotion, development and dissemination of integrity and content of the speciality of Haematology and haemotherapy in its aspects medical, scientific, organizational, health care, teaching and research. Likewise, arises the need to approach to society in generalin such a way that be perceived and properly understand the value of the professional role of the hematologist and its competence in medicine. Its central axis consists of 17 cooperative groups that currently are, each focused on a specific aspect of the specialty. With 53 years of life, the AEHH is today a powerful organization with a considerable scientific impact, since many of the more than 2,000 professional Hematologists as part of her are internationally recognized figures for many years thus becoming one of the most visible parts of Spanish medicine abroad.
